Clean-up Report Form
Please use the form below to submit your clean-up activity and submit one report per location, per day of clean-up.
Please call (940) 498-7501 for trash bags to be collected. Be sure to include the location and number of bags collected.

Use safety vests and gloves Wear long pants, long-sleeved shirts, closed-toe shoes, hat, sunglasses, sunscreen, and bug spray Work in teams to maximize safety Stay hydrated Pick up micro-litter such as cigarette butts and coke can tabs Tie garbage bags and place away from sidewalk and street Know the emergency procedures, such as the location of the nearest emergency facility and how to summon an ambulance or the police to your location Be aware of your surroundings and potential hazards – passing cars, hazardous materials, tree branches, poison ivy, snakes, bees, etc. Wash hands with soap after cleanup |
Pick up broken glass, rusty metal objects, tree branches, or heavy pieces of wood* Pick up hazardous materials, such as hypodermic needles, sharp objects, old car batteries, animal carcasses, oil containers, chemicals, or chemical containers, 55-gallon drums, or any other unidentified or questionable object* Attempt to move large objects Allow children to participate unless supervised by an adult Perform any activities outside of your physical capabilities Conduct cleanups during inclement weather Schedule cleanups during peak pedestrian or traffic hours Conduct cleanups near or around construction sites
